Intermediate Care

What is Intermediate Care?
Intermediate Care is a short term service provision up to 6 weeks which provides rehabilitation within peoples own homes throughout the borough of Stockport.


The service is provided from 7am until 10pm, 7 days a week, but overnight support can be arranged if necessary.

The Team
The Intermediate Care team consists of a dedicated team of staff, consisting of multi skilled workers and Assistant Practitioners’ with social, clinical and therapy skills.


The Home Support Coordinators are trained Key Workers who identify specific moving and handling requirements and work closely with the support staff.

Aim of the service
The aims of the Intermediate Care service are:

  • To ensure people achieve their maximum potential
  • To enable people to remain as independent as possible within their own homes
  • To ensure all needs are met with positive outcomes

An assessment of your individual needs
In order to meet specific needs a personal support plan will completed on the first visit by a Home Support Coordinator. This enables us to focus on the outcomes people would like to achieve from the service we provide.

An environmental risk assessment is also carried out, to ensure safety for both our users and our staff. Aids and adaptations are identified and put in place.
